Requirements:

• Baccalaureate degree with an emphasis in Speech-Language Pathology or Audiology
• 25 hours of clinical observation
• 25 hours of clinical assisting experience
• 24 semester hours in speech-language pathology and audiology with at least 18 in the area in which the applicant is applying

Those requesting verification of their 25 hours of clinical observation must submit the following to Dr. Diana Gonzales:

• Check in the amount of $25 made payable to the CDIS Foundation
• Completed Hours Form

If the Name, SS#, and University Name lines of the Verification of Hours Form are not completed by you, then your hours will not be processed. Thank you for your assistance in making it as easy as possible to help you in a timely manner.
