Application to the Department of Physical Therapy

Two applications are required. Both are due on October 15, 2008. If not complete at that time, applications will not be considered.
Application is a two (2) part process:
1. Graduate College
2. Department of Physical Therapy
The following items are to be submitted to the Graduate College:
- An official Graduate College Application for Admission by October 15th, 2008.
- A non-refundable application fee of $40.00 (check or money order) to the Graduate College.
- Evidence of a completed bachelor’s degree from an accredited university.
- Official transcripts from all institutions attended.
- A preferred Graduate Record Exam (GRE) of 1000 or higher, verbal and quantitative combined. This score must be on file in the Graduate College prior to the evaluation of your application.
- All supporting documentation must be in the Graduate College Office by October 15th for the application to be considered complete.
The following items are to be submitted to the Department of Physical Therapy:
- The completed Department of Physical Therapy Application by October 15th, 2008.
- A non-refundable application fee of $25.00 (money order only) to the Department of Physical Therapy.
- All completed application forms – application form, pre-requisite form, reference checklist forms (3), and support statements (2).
- Transcripts are not required with the PT application, only with the Graduate College application.
- All supporting documentation must be in the PT Department Office by October 15th for the application to be considered complete.
