Junior-Senior Sequence Application

Please contact the College of Health Professions Advising Center at (512) 245-3506 to make an appointment prior to submitting your application to the department. Fill out the Pre-CDIS Tracking Form, and the CDIS undergraduate application, online - as these will be reviewed during your appointment (we recommend using the browser Mozilla Firefox). After your appointment, these two forms will be submitted via email to the CDIS Department. The deadline to submit an application is May 15.
